It is the responsibility of the pharmacy to make sure that you receive the proper information about the medication that you receive, so you can be sure that the medication is used safely; for its use and within its restrictions.
To do our job well we need to receive accurate information from you. Not just your date of birth and your gender but also your allergies, pregnancy, breast feeding and other medicines that you use or what you are being treated for. This ensures safe use of medication. It is team work!
The patient is responsible for providing accurate information and for updating any outdated information about personal status, health and or any extra medication that was started, especially in the case of repeat prescriptions. If our information is not correct or incomplete the pharmacy cannot perform its job in providing safe use of medication.
